Below are links to several different forms used by our student ministry.
You
can fill them out online and then print them.
Most have to be signed
and/or notarized, so they will need to physically be brought into the student ministry office (or mailed in).
2008 Medical Release Form
Required
to be filled out by every student attending any special function. This
includes lock-ins, retreats, camp, etc. You only need to fill this out
once a year, and it will cover all events. It needs to be notarized.
Scholarship Application
FBC
never wants anyone to be unable to attend an event because of a lack of
financial resources. If you have a special situation and need
financial aid, we may provide assistance for you. You need to fill out
an application, and the office will contact you. All information will
be kept confidential.
